Every dental provider wants to be compensated for the services they’ve performed on their patients in a timely fashion. So, we’ve developed some tips on what you need to do to ensure prompt and accurate payment. Please include the following information when submitting your claim to Cigna Dental:
- Policyholder/Subscriber Name and ID number
- Treating dentist name, license number, NPI
- Dental office Tax ID number or treating dentist’s Social Security Number
- Address where services were rendered
- Date of service, procedure code(s), procedure description(s), tooth number(s), surface(s), quadrant(s), location or arch
On the claim form, you should input your Usual and Customary Fee (not your contracted Cigna fee if you are in-network) in the Fee column.
Claim Submission Methods
All claims should be submitted to the Dental Plan using one of the following methods:
> Electronically, through a clearinghouse that provides an electronic interface with third-party Payers
> Via a standard ADA Dental Claim Form (any year acceptable)
> Via a Dental Plan Claim Form (if available)
> Forms that any state regulations may require

Coordination of Benefits
If you are aware that the patient has dental coverage through more than one plan, please submit your Claim to the primary carrier first and then submit a Claim Form with a copy of the primary carrier’s Explanation of Benefits (EOB) to the secondary carrier. When Cigna is primary, the benefit will be paid without regard to payments from the secondary carrier. Coordination of Benefits described in the Dental Plan certificate booklet will apply when Cigna is the secondary carrier. If the patient has coverage under their Medical plan for Oral surgery then you should submit a copy of the medical EOB with your claim form.
