While completing claim forms correctly is essential, one of the most common reasons for claim delays is the absence of required supporting documentation, particularly X-rays, narratives, Explanation of Benefits (EOBs), and other clinical attachments. Providers who submit complete documentation up front can help reduce administrative follow-up, improve claims accuracy, and accelerate payment processing.

Why Attachments Matter

Many dental procedures require additional clinical documentation before benefits can be reviewed and processed. Missing X-rays, narratives, or supporting records can result in requests for additional information, which may extend processing timelines and create unnecessary administrative work for dental offices.

Submitting required attachments with the initial claim helps ensure the claim can be reviewed efficiently and reduces the likelihood of avoidable delays.

X-Rays: A Critical Component of Many Claims

For certain dental procedures, X-rays provide the clinical documentation needed to support treatment and benefit adjudication. Providers should review current X-ray submission requirements and ensure that all applicable radiographs are included with each claim, even if X-rays have been submitted with any prior claims. X-rays must be of diagnostic quality and should be labeled with the patient’s name and date the x-ray was taken.

Best Practices for Faster Claim Processing

  1. Submit complete clinical documentation upfront. Before transmitting a claim, verify whether the procedure requires X-rays, photographs, narratives, treatment plans, or other supporting documentation.
  2. Use electronic submission whenever possible. Electronic claims and electronic attachment submissions can support faster delivery, improved tracking, and fewer missing information issues than paper processes.
  3. Verify coordination of benefits documentation. When another dental or medical plan has already processed a claim, submit the applicable EOB along with the secondary claim.
  4. Ensure specialty claims include required details. Orthodontic claims should include complete fee itemization, treatment descriptions, appliance information, and treatment duration details.
  5. Review regulatory requirements. For Medicare-covered dental services associated with covered medical procedures, ensure required documentation and modifiers are included and properly reported.
