To update your contact information, connect with us by using one of the methods below. Please type or print neatly, and provide your Taxpayer Identification Number (TIN) to help us accurately identify your practice.

- Download a Dentist Change Form (complete electronically then email, fax or mail to Cigna as indicated) and submit with any required documentation. If your change requires it, please submit a W-9 Form.
